Description

We are seeking an experienced Home Health Registered Nurse (RN) for a 13-week contract assignment providing in-home patient care services. This role is ideal for nurses with strong home health, medical-surgical, and orthopedic experience who are passionate about delivering compassionate patient-centered care.

Responsibilities:

Provide skilled nursing care to adult and geriatric patients in home settings
Complete admission assessments, follow-up visits, and discharge planning
Perform wound care, IV therapy, phlebotomy, and medication management
Care for post-surgical, orthopedic, stroke, and chronic illness patients
Educate patients and families on medications, treatments, wound care, trach care, and feeding tubes
Utilize EPIC charting and maintain accurate documentation
Travel between patient homes within assigned coverage territory
Participate in after-hours and weekend on-call rotation as needed

Requirements: Minimum 1 year of Home Health RN experience required 1–2 years of Medical-Surgical or acute care experience required Orthopedic experience required Active RN license required BLS certification required Strong phlebotomy, IV therapy, wound care, and patient education skills OASIS documentation experience preferred Excellent communication and time management abilities
