Pride health is hiring a Registered Nurse for our client's home health program in Lewistown, PA 17044.

Job Title: Travel RN – Home Health

Contract Details

• Weekly Hours Guaranteed: 36 Hours
• Shift Schedule: Monday – Friday | 8:00 AM – 4:30 PM
• On-Call/Weekend Requirement: Yes
• Contract Type: Travel Assignment (Duration: 6 Months contract + Extension based on business needs)

Compensation: (Local Pay range: $53.00 to $57.00 per hour)

Travel Package:

• Weekly Pay: $2,200 – $2,586
• Lodging Stipend: $770/week
• M&IE Stipend: $476/week

Mileage reimbursement is provided between patient visits.

Coverage Area: Mifflin & Juniata Counties, PA

Position Overview:

We are seeking an experienced and compassionate Home Health Registered Nurse (RN) to join a leading home health organization in Lewistown, PA. The ideal candidate will provide skilled nursing care to patients in their homes while ensuring high-quality patient outcomes and compliance with home health standards.

Key Responsibilities

• Perform comprehensive nursing assessments and patient evaluations.
• Complete admissions and start-of-care visits.
• Provide patient and caregiver education.
• Accurately complete OASIS documentation.
• Develop and implement individualized care plans.
• Coordinate care with physicians and interdisciplinary healthcare teams.
• Monitor patient progress and update treatment plans as needed.
• Maintain timely and accurate clinical documentation.

Required Qualifications

• Active Registered Nurse (RN) license.
• Home Health nursing experience preferred.
• Strong OASIS documentation skills.
• Excellent clinical assessment and communication skills.
• Ability to work independently and manage travel within coverage areas.

Preferred Qualifications

• Prior experience in Home Health admissions.
• Strong patient education background.
• Familiarity with productivity-based environments.

• Productivity Expectation: 30 points/week
