Physical Therapist (PT) – Home Health
Danville, KY | 13-Week Travel Contract
Ardor Health Solutions is seeking an experienced Home Health Physical Therapist for a contract opportunity with CenterWell Home Health in Danville, Kentucky. This position offers the autonomy of home health, one-on-one patient care, and the opportunity to make a meaningful impact in patients' lives while enjoying the charm of central Kentucky.
Assignment Details
- Specialty: Physical Therapist (PT)
- Setting: Home Health
- Schedule: Monday–Friday, 40 hours per week
- Contract Length: 13 weeks
- Start Date: ASAP
Requirements
- Active Kentucky PT license
- Minimum 1 year of Home Health experience required
- Oasis experience required
- Home Care Home Base (HCHB) experience required
- Strong clinical assessment and documentation skills
- Ability to work independently and manage a territory
