Job: Registered Nurse – Case Manager
Location: Santa Rosa, CA
Duration: 13 Weeks
Shift: Day Shift | 5x8 Hours (8:00 AM – 4:30 PM)
Start Date: 06/29/2026
Job Description:
• Seeking an experienced RN Case Manager for an acute care hospital setting
• Minimum 2 years of acute hospital Case Management experience required
• Must have recent work history within the last 2 years
• Experience in care coordination and discharge planning in a fast-paced environment required
• EPIC charting experience required
• Previous travel experience preferred, but first-time travelers may be considered with clinical approval
• BLS certification required
• Experience with HMOs, IPAs, DME coordination, SNF placement, and ambulance transportation arrangements required
• Must be comfortable working independently with discharge planning and patient care coordination
