Position Overview

Ardor Health Solutions is seeking an experienced Home Health Physical Therapist for a 13-week travel assignment with Amedisys Home Health in Garden City, New York. This position offers the opportunity to provide personalized, one-on-one care that helps patients regain mobility, improve function, and remain safely in the comfort of their own homes while working alongside a supportive interdisciplinary team.

Job Responsibilities

• Complete comprehensive physical therapy evaluations in patients' homes.
• Develop individualized treatment plans based on each patient's goals and functional needs.
• Treat patients recovering from orthopedic procedures, neurological conditions, cardiopulmonary diagnoses, and generalized weakness.
• Educate patients and caregivers on home exercise programs, mobility, safety, and fall prevention.
• Collaborate with physicians, nurses, occupational therapists, speech therapists, and case managers.
• Complete accurate and timely documentation using Homecare Homebase (HCHB).
• Travel throughout the assigned coverage area to provide patient care.

Qualifications

• Active New York Physical Therapist license (or ability to obtain before start)
• Minimum of 1 year of recent Home Health experience
• OASIS documentation experience required
• Homecare Homebase (HCHB) experience strongly preferred
• Current BLS certification
• Valid driver's license, reliable transportation, and current auto insurance
• Strong communication and organizational skills
