• Assess, evaluate, and document patient wounds, skin integrity, and healing progress.
• Develop and implement individualized wound care treatment plans based on patient needs and physician orders.
• Perform wound care procedures, including dressing changes, debridement assistance, negative pressure wound therapy (wound VAC), and ostomy care.
• Monitor wound healing outcomes and modify care plans as appropriate.
• Collaborate with physicians, surgeons, physical therapists, dietitians, and interdisciplinary healthcare teams.
• Educate patients, families, and caregivers on wound management, prevention strategies, and home care techniques.
• Identify and implement measures to prevent pressure injuries and skin breakdown.
• Maintain accurate and timely documentation of wound assessments, treatments, and patient responses.
• Collect wound cultures and monitor laboratory results relevant to wound healing.
• Ensure compliance with infection prevention, safety standards, and regulatory requirements.
• Provide consultation and guidance to nursing staff regarding wound care best practices.
• Participate in quality improvement initiatives focused on wound prevention and healing outcomes.
• Evaluate medical supplies and recommend appropriate wound care products and equipment.
• Monitor patients for signs of infection, complications, or delayed healing and report findings promptly.
• Support discharge planning and coordinate continuity of wound care services.

Typical Requirements

• Active Registered Nurse (RN) license.
• Clinical nursing experience; wound care experience preferred.
• Certification such as WCC, WOCN, CWCN, or equivalent preferred.
• Knowledge of pressure injury prevention, ostomy care, and advanced wound treatment modalities.
• Strong assessment, patient education, and interdisciplinary collaboration skills.
