Description

Overview

Seeking a compassionate and experienced Long Term Care (LTC) RN to provide skilled nursing care to residents in a long-term care setting. The ideal candidate will be dedicated to delivering high-quality, resident-centered care while collaborating with an interdisciplinary team.

Responsibilities

Assess, plan, implement, and evaluate resident care plans
Administer medications, treatments, and nursing interventions
Monitor resident conditions and report changes as appropriate
Coordinate care with providers, families, and healthcare team members
Maintain accurate and timely documentation
Supervise and support LPNs and CNAs as needed

Requirements

Active RN License
Minimum 1 year of LTC, Skilled Nursing, Rehab, or Acute Care experience
Strong assessment, communication, and organizational skills
Ability to manage multiple residents in a fast-paced environment
