Contract Length: 12 Weeks (Potential for extension)
Schedule: Monday–Friday, 8:00 AM – 5:00 PM
Shift: Days (40 Hours/Week)

Position Overview

McAlester Regional Health Center is seeking an experienced Registered Nurse (RN) Case Manager for a 12-week travel assignment. This is an excellent opportunity for a skilled case management professional with a strong background in utilization review, discharge planning, and inpatient care coordination.

The RN Case Manager will work collaboratively with physicians, nursing staff, patients, families, and community resources to ensure appropriate utilization of healthcare services while facilitating safe and efficient patient transitions throughout the continuum of care.

Key Responsibilities

• Coordinate and manage patient care throughout hospitalization.
• Perform comprehensive case management assessments and develop individualized care plans.
• Conduct utilization review to ensure appropriate level of care and resource utilization.
• Facilitate discharge planning and coordinate post-acute care services.
• Collaborate with interdisciplinary healthcare teams to optimize patient outcomes.
• Identify barriers to discharge and proactively implement solutions.
• Ensure compliance with regulatory, payer, and facility requirements.
• Educate patients and families regarding treatment plans, discharge needs, and community resources.
• Maintain accurate and timely documentation within the electronic medical record.

Required Qualifications

• Active Registered Nurse (RN) license.
• Minimum 3–5 years of Case Management experience.
• Strong experience in Utilization Review and Discharge Planning.
• Recent inpatient hospital case management experience required.
• Excellent communication, organization, and critical-thinking skills.
• Ability to manage multiple patient cases in a fast-paced healthcare environment.

Assignment Details

• Inpatient Case Management Department
• Average daily census approximately 40 patients
• Collaborative team environment with three Case Managers on staff
• No call requirements specified
• Day shift schedule with weekends off
