Description
- Conduct comprehensive patient assessments to evaluate medical needs, treatment plans, discharge requirements, and appropriate levels of care.
- Perform utilization review activities to ensure services are medically necessary, cost-effective, and compliant with payer guidelines and regulatory requirements.
- Coordinate patient care across multidisciplinary teams, including physicians, nurses, therapists, social workers, and insurance providers.
- Develop and implement individualized care plans to improve patient outcomes, reduce readmissions, and facilitate timely transitions of care.
- Monitor patient progress and resource utilization, identifying barriers to care and recommending appropriate interventions.
- Facilitate discharge planning and care transitions, ensuring patients have access to necessary post-acute services, equipment, medications, and community resources.
- Maintain accurate documentation and communicate with insurance companies to obtain authorizations, support appeals, and ensure compliance with healthcare regulations and organizational policies.
