13 Weeks contract
40 Guaranteed hours
$2550/-
No Locals.

Shift:

8:00 AM – 6:30 PM (10-hour days)
40 hrs/week | Rotating schedules (Thu–Sun / Sat–Tue)

Requirements:

1–2 years Home Health & Hospice experience (required)
Recent OASIS experience (required)
BLS, WI RN License, Driver’s License
EPIC experience preferred

Visits:

1:1 patient visits
~7–7.5 visits per day

Patient Type:

Adults: CHF, COPD, Diabetes, Cancer, CVA
Wound care, IV therapy, hospice symptom management

Setting:

Home Health + Hospice (field visits)
