SimpliFi Clinical Interviewing- Yes/No- Yes

On Call, Call Back Requirements- 3 call shifts 9p-7am

Weekend Rotation- Every other Saturday

S Sub - Hemodialysis

• 17800 Kedzie Ave. Hazel Crest, IL 60429

• 30 min from Chicago

• 231 Beds

• No trauma designation

• Teaching Hospital

• General Information

• Beds: 6 bays

• Experience? 5+ years of Dialysis experience

• First-time traveler? No

• Patient Types:

• Hemodialysis patients, including portable to bedside in ICU

• Skills required

• Inpatient acute dialysis experience, able to read and interpret cardiac rhythm strips, experience with Fresenius T or K machines, EPIC experience

• Patient Ratios: 1:2 in the Dialysis unit and 1:1 in the ICU

• Required Certifications:

• Required: BLS

• Preferred: ACLS

• Support on the Unit:

• Charge nurse: may take patient assignment

• Phlebotomy? Yes

• IV/VAT Team available for hard sticks, PICC lines, midlines, etc.

• Resource Nurse: is available at night and floats between units

• RT in house

• Hospitalist in house

• OT, PT, ST during the week

• Charting System/Equipment:

• EPIC - required

• Alaris

• Pyxis

• Floating:

• Can travel to the ICU to dialyze patients

• Orientation:

• One shift to learn flow

• Scheduling:

• 0700 start time

• Weekend rotation: every other Saturday

• Can we approve time off? Manager to approve

• Holiday Expectations: every other

• Call: 2 shifts per week 0730-1900; 1 Sunday per month

• 30 min response time

• Will NOT accommodate block scheduling

• Parking: Employee parking, free.

• No artificial fingernails or extenders when having direct patient care

• Keep natural nail tips less than ¼ inch long
