Schedule: Monday–Friday, 8:00 AM – 5:00 PM


Requirements:

Active RN license
Recent hospice nursing experience (required)
Experience with Homecare Homebase (HCHB) EMR system (required)
Strong patient assessment and care planning skills
Experience providing end-of-life and palliative care
Ability to coordinate care with interdisciplinary teams
Experience educating and supporting patients, families, and caregivers
Knowledge of hospice regulations, documentation standards, and compliance requirements
Excellent communication and organizational skills
