📋 Role Overview:
• Provide home health physical therapy evaluations and treatments
• Develop and implement individualized care plans
• Work closely with physicians and interdisciplinary teams
• Educate patients and caregivers on care plans
• Complete documentation and OASIS assessments

✅ Requirements:
• Active PT license
• Home health experience preferred
• OASIS experience required
• Strong assessment and care planning skills

• Mileage reimbursement provided
• Productivity expectation: ~30 points/week
• Coverage includes multiple counties
• In‑person interview required
