Description
REQUIRED: - ACTIVE License - LMSW or LCSW AND ACTIVE MS State License & BLS
Minimum 2 years of Acute Inpatient Case Management experience that includes at least 1 year in a LARGE Hospital (500+ beds, LEVEL 1/ACADEMIC Medical Center)
Previous travel in a level 1 facility- Strong computer skills, Epic EMR experience, - Acute care/ED discharge planning experience, Recent (within the last two years) acute inpatient case management experience
