Job Details:

RN Care Manager – Home Health
Location: Simi Valley, CA 93065
Contract Length: 16 Weeks
Start Date: June 29, 2026
End Date: October 18, 2026
Shift: Days (Monday–Friday)
Schedule: 8:00 AM – 5:00 PM
Hours: 40 Hours per Week

Weekend Requirement: Rotating weekends included within the 40-hour schedule
Float Requirement: Must be willing to assist at Glendale location as needed

Job Summary:

Adventist Health Simi Valley is seeking an experienced RN Care Manager for a Home Health assignment. The RN Care Manager will coordinate and manage patient care services, collaborate with interdisciplinary teams, and ensure quality outcomes for patients receiving home health services.

Requirements

Active California RN License
Current BLS/CPR Certification
EPIC EMR experience required
Valid Driver’s License
Reliable Vehicle
Current Auto Insurance
Must be at least 21 years of age
