Pride Health is hiring experienced and compassionate Home Health Registered Nurse (RN) to join a leading home health organization in  Louisville, KY. The ideal candidate will provide skilled nursing care to patients in their homes while ensuring high-quality patient outcomes and compliance with home health standards.

Duration: 13-week Contract (Possibilities of extension)

Schedule: Mon – Fri (5 days/wk.) from 8 AM to 4:30 PM 
 
On Call/ weekend Requirement: One weekend on call/month
Mileage: Reimbursement provided (from first patient to last patient)
 
Job Responsibilities:

• Perform comprehensive nursing assessments and patient evaluations.
• Complete admissions and start-of-care visits.
• Provide patient and caregiver education.
• Accurately complete OASIS documentation.
• Develop and implement individualized care plans.
• Coordinate care with physicians and interdisciplinary healthcare teams.
• Monitor patient progress and update treatment plans as needed.
• Maintain timely and accurate clinical documentation.

 
Qualifications:

• Active Registered Nurse (RN) license.
• Home Health nursing experience.
• Strong OASIS documentation skills.
• Excellent clinical assessment and communication skills.
• Ability to work independently and manage travel within coverage areas.
