Job Title: Visiting Home Health RN
Location: Waterford WI 53185
Coverage area: Waukesha, Milwauke, Washington, Ozaukee counties
Work Schedule: M-F 8am-5pm (No on call)
Pay: $54.00 - $56.00 per hour (hourly pay - not per visit) 
Scheduled pay: Weekly
Milage: 0.53 cents per mile
Benefits: Health, Dental, Vision, Life Insurance, and Short term disability after 90 days
Job Type: Full time – W2 contract to hire opportunity

Responsibilities 

• Waukesha, Milwauke, Washington, Ozaukee counties
• Nurse will only perform patient visits (assessments, wound care, medication reviews, education, etc.)
• Collaborate with primary care providers to ensure the implementation of an individualized, comprehensive care plan with specific interventions designed to engage the member.
• Address, identify, and continuously reassess cost-efficient, appropriate levels of care. Put non medical support in place to ensure compliance with treatment plans (such as housing and transportation).
• Coordinate care transitions as needed. Coordinate with physicians, social workers, discharge planners, and claims professionals in transitioning patient to appropriate level of care.
• Engage specialty resources as needed to achieve optimal resolution.
• Keep detailed records of clinical, functional, and fiscal outcomes during the management process. Identify opportunities for health promotion and illness prevention.
• Prevent adverse patient occurrences when possible and intervene quickly if prevention is not possible, thereby minimizing poor outcomes.
• Act as patient advocate by protecting privacy and confidentiality issues. 

Required Skills & Qualifications 

• Active RN license in the state of Wisconsin 
• 1 year home health experience - patient assessments, wound care, medication reviews, and patient/family education
• homecare homebase experience -HIGHLY PREFERRED 
• Matrix Care is acceptable or any Electronic Medical Record related to home health - required
