Job Summary – RN Case Manager

The RN Case Manager coordinates and manages patient care across the healthcare continuum, ensuring quality, cost-effective, and patient-centered services. They collaborate with patients, families, physicians, and interdisciplinary teams to develop, implement, and monitor individualized care plans.

Roles & Responsibilities

Assess patients' medical, psychosocial, and discharge planning needs.
Develop and coordinate individualized care plans.
Monitor patient progress and adjust care plans as needed.
Facilitate communication among healthcare providers, patients, and families.
Coordinate referrals, community resources, and post-discharge services.
Ensure compliance with healthcare regulations, policies, and documentation standards.
Advocate for patients to achieve optimal health outcomes and continuity of care.
Educate patients and families regarding treatment plans and available resources.

Documents Required:

Valid NY State RN license
At least 2 years of experience.
DISCHARGE EXPERIENCE REQUIRED
BLS Certified through AHA
