Travel Physical Therapist (PT) - Home Health

Join a leading Home Health provider for a rewarding travel Physical Therapist opportunity in the Denver metro area. This assignment offers the chance to work independently while helping patients regain mobility, improve function, and remain safely in their homes.
You'll work with a diverse adult and geriatric patient population throughout the Denver area, providing evaluations, treatment plans, patient education, and ongoing care coordination with physicians and interdisciplinary team members.

Coverage varies based on census and scheduling needs.
Responsibilities
- Perform home health physical therapy evaluations
- Develop and implement individualized treatment plans
- Assess patient mobility, strength, balance, and functional limitations
- Provide therapeutic exercise and mobility training
- Educate patients and caregivers on safety and home exercise programs
- Coordinate care with physicians, nursing, and interdisciplinary team members
- Complete timely documentation and OASIS assessments
