Description
Schedule & Assignment Details
- Shift: Monday – Friday, 8:00 AM – 5:00 PM
- Hours: 40 Hours per Week
Key Responsibilities
- Provide skilled nursing care to patients in their homes.
- Conduct comprehensive patient assessments and develop individualized care plans.
- Complete and maintain OASIS documentation and charting in HCHB (Homecare Homebase).
- Perform case management duties and coordinate care with physicians, therapists, and other healthcare providers.
- Monitor patient conditions and report changes to physicians promptly.
- Manage medication administration, reconciliation, monitoring, and education.
- Ensure compliance with agency policies, state regulations, and home health standards.
- Supervise and direct Licensed Practical/Vocational Nurses (LPNs/LVNs) as needed.
- Coordinate health-related appointments and follow-up care.
- Communicate with hospital discharge planners and inpatient care teams during transitions of care.
- Complete daily, monthly, quarterly, and annual nursing documentation.
- Provide patient and caregiver education on disease management, health maintenance, and safety.
- Conduct new employee training and annual competency reviews related to health and safety.
- Promote infection prevention and maintain safe home care environments.

- Strong OASIS experience required.
- Homecare Homebase (HCHB) experience required.
- Experience with case management and care coordination.
- Ability to work independently in community-based settings.
