Job Summary
An advanced regional trauma center in South Weymouth is seeking an elite, highly resilient Critical Care RN for a 13-week night assignment. This high-velocity ICU/CCU environment demands an intensive care specialist capable of managing an extremely diverse patient population. The incoming nurse must possess the clinical acumen to provide continuous, hyper-vigilant assessments (Q1H or greater) and immediate life-support interventions for critically ill patients across multiple specialized domains.
Key Responsibilities
- Multi-Specialty Critical Care: Deliver comprehensive, high-acuity nursing care to an expansive patient demographic, including Level II trauma victims, complex post-surgical cases, high-risk obstetric emergencies, acute neurovascular events (strokes/ICH), and medically unstable cardiac or septic patients.
- Hyper-Vigilant Monitoring: Execute continuous, rigorous physical, neurological, and hemodynamic assessments at minimum Q1H frequencies, tracking subtle physiological shifts to preempt clinical decompensation.
- Advanced Vasoactive Titration: Independently initiate, calculate, and aggressively titrate high-alert life-support infusions (e.g., Norepinephrine, Epinephrine, Propofol, Insulin protocols) based on precise targeted parameters.
- Mechanical Airway Management: Safely manage, troubleshoot, and wean intubated, mechanically ventilated patients, maintaining strict adherence to ventilator-associated pneumonia (VAP) prevention bundles.
- Invasive Line Navigation: Access, maintain, and accurately interpret data from advanced invasive monitoring equipment, including arterial lines, central venous catheters, and continuous telemetry interfaces.
Requirements & Qualifications
- Acuity Adaptability: Verifiable background managing a dynamic cross-section of trauma, neurological, and post-operative surgical ICU populations.
- Technical Proficiency: Standalone competency with advanced critical care hardware (e.g., rapid infusers, specialized cooling/warming blankets, point-of-care laboratory devices).
- Documentation Standards: Meticulous, real-time electronic charting of intensive care flowsheets, tracking precise intake/output fluid balances and medication titration histories.
Unit & Facility Details
- Department Environment: SSH Critical Care HB. This unit manages highly vulnerable, acutely ill patients requiring immediate, continuous resource-intensive nursing care.
- Schedule Precision: Straight night rotation (19:00 – 07:30). Expect a standard weekend and holiday rotation native to a 24/7 critical care ecosystem.
- Coordination Contact: Kelsey Rodriguez.
Clinical Spotlight: Managing the Critically Ill Obstetric Patient in the ICU
One of the most complex assignments on this cross-functional critical care unit is the management of a severe postpartum obstetric emergency, such as Preeclampsia with Severe Features or HELLP Syndrome.
When a patient is transferred directly from Labor & Delivery due to severe hypertension and impending eclampsia, the critical care nurse must execute a precise, dual-system balancing act:
- Neurological Safeguards: Safely initiate and manage a continuous Magnesium Sulfate infusion (typically a $4\text{–}6\text{ g}$ loading dose followed by a $2\text{ g/hr}$ maintenance dose) to elevate the seizure threshold.
- Toxicity Tracking: Conduct strict hourly checks for signs of magnesium toxicity—proactively monitoring for the loss of deep tendon reflexes ($DTRs$), a drop in respiratory rate below $12\text{ breaths/min}$, and urine output dropping below $30\text{ mL/hr}$. The nurse must keep Calcium Gluconate ($1\text{ g}$ IV over 5–10 minutes) readily available at the bedside as the immediate rescue antidote.
- Targeted Antihypertensive Titration: Co-manage blood pressure via titratable continuous IV infusions such as Labetalol or Nicardipine, safely lowering mean arterial pressure ($MAP$) without causing rapid perfusion drops that could compromise maternal organ health.
