The Licensed Clinical Social Worker collaborates with the multi-disciplinary team to provide input in the development of the plan of care for those patients/clients requiring social work intervention.  The Licensed Clinical Social Worker provides counseling and crisis intervention to patients/clients who have social or emotional problems in a variety of settings across the age span.  Minimum Requirements: <ul><li>Master’s degree in Social Work from a school of social work accredited by the Council on Social Work Education required</li><li>Active license as an LCSW in state of practice required</li><li>Minimum of one year of social work experience preferred</li><li>Current CPR if applicable</li><li>TB questionnaire, PPD or chest x-ray if applicable</li><li>Current Health certificate (per contract or state regulation)</li><li>Must meet all federal, state and local requirements</li><li>Must be at least 18 years of age </li></ul> General CommentsLCSWMonday-Friday8a-5p
