Description
Contract Length: 13 Weeks (06/29/2026 – 09/25/2026)
Schedule: Monday–Friday, 8:00 AM – 4:30 PM
Hours: 40 Hours/Week (Guaranteed 36 Hours)
Productivity Requirement: 30 points
Mileage Reimbursement: Yes (between visits, first to last patient)
Coverage Area: Lucas, Wood, Fulton, Hancock, Henry, and surrounding counties
EMR: Homecare Homebase (HCHB) – preferred/expected
Key Responsibilities
- Perform comprehensive home health assessments and skilled nursing visits.
- Provide case management for assigned patients in coordination with the interdisciplinary team.
- Develop, implement, and update individualized Plans of Care (POC).
- Complete OASIS assessments accurately and in compliance with regulatory guidelines.
- Coordinate care with physicians, therapists, and other healthcare professionals.
- Educate patients and caregivers regarding disease management, medications, and treatment plans.
- Monitor patient status and report changes in condition to the physician and care team.
- Ensure timely and accurate documentation in Homecare Homebase (HCHB).
