Job Title: Inpatient Care Manager (RN) – Utilization Review & Discharge Planning
Location: Lincoln Hospital, New York, NY
Duration: 2-3 Months
Schedule: Monday–Friday | 9:00 AM – 5:00 PM (35 Hours/Week)
Job Description:
We are seeking an experienced Registered Nurse Care Manager to support its Inpatient Care Management team.
The ideal candidate will have a strong background in Care Coordination, Utilization Review, and Discharge Planning within an acute care hospital setting.
Key Responsibilities:
- Coordinate inpatient care management activities.
- Perform utilization review and ensure appropriate level of care.
- Develop and facilitate discharge plans for patients.
- Collaborate with interdisciplinary healthcare teams.
- Maintain accurate documentation and care management records.
- Support patient-centered care and resource utilization initiatives.
