Description
Nurse Supervisor
Location: New York, NY
Employment Type: Acute - Nursing
Shift: Provide Associate Director level supervision to Care Management Department
Hours: 7 hours/day, 37.5 hours/week
Contract Length: 14 weeks
Job Summary
- Assist with planning, organizing, directing, coordinating, and evaluating nursing services and educational programs within transitional care.
- Collaborate with the transitional care team to reduce 30-day readmissions and support high-risk patients post-discharge from inpatient med/surg units and Adult Emergency Department.
- Organize and oversee care coordination to optimize patient health in the community.
- Perform duties of the Director of Nursing in their absence.
- Develop policies and standards for nursing service and education in care management and transitional care.
- Provide oversight and direction to Accountable Care Managers (RNs), Care Coordinators, and ancillary staff.
- Provide Associate Director level supervision to the Care Management Department.
Required Qualifications
Experience
- Minimum 2 years of care management experience, including experience within the last year.
- At least 2 years of experience in discharge planning and utilization review.
- Experience in supervisory roles.
- Med Surg experience.
Education
- Bachelor of Science in Nursing from an accredited program required.
Certifications & Licensure
- Valid Registered Nurse (RN) license in New York with primary source verification.
- Certified Case Manager (CCM) certification required.
- Basic Life Support (BLS) certification required.
- PRI certification required.
