Travel Registered Nurse (RN) – Case Management
Location: Dallas, TX
Schedule: Saturday – Monday | 7:00 AM – 7:00 PM (36 Hours/Week)
Employment Type: Travel Contract
Job Summary
We are seeking an experienced and compassionate Registered Nurse (RN) Case Manager to join its care management team. The RN Case Manager will be responsible for coordinating patient care throughout the continuum, ensuring appropriate utilization of healthcare resources, facilitating discharge planning, and promoting positive patient outcomes.
Key Responsibilities
- Coordinate and manage patient care from admission through discharge.
- Develop and implement individualized care plans.
- Collaborate with physicians, nurses, social workers, and interdisciplinary teams.
- Perform utilization review and assess medical necessity of services.
- Facilitate discharge planning, including referrals to rehabilitation facilities, skilled nursing facilities, home health, and community resources.
- Educate patients and families regarding treatment plans and post-discharge care.
- Ensure compliance with regulatory requirements and hospital policies.
- Maintain accurate and timely documentation in the electronic medical record.
Qualifications
- Active Registered Nurse (RN) license.
- Minimum one (1) year of recent Case Management experience in an acute care hospital setting.
- Strong knowledge of discharge planning, care coordination, and utilization review.
- Excellent communication, organizational, and critical-thinking skills.
- Ability to work effectively in a fast-paced healthcare environment.
