Description

Travel LVN/LPN – Home Health
Location: Manchester, NH
Shift: Days – 5x8 Hours (8:00 AM – 4:30 PM)
Guaranteed Hours: 40 Hours/Week

Job Summary

We are seeking an experienced LVN/LPN Home Health Nurse to provide skilled nursing care to adult and geriatric patients in their homes. This role involves managing a variety of medical and surgical conditions while delivering high-quality, patient-centered care in a home health setting.

Responsibilities

Provide home health nursing care to adult and geriatric patients.
Perform wound care, wound vac management, and ostomy care.
Manage indwelling catheters and post-surgical patients.
Perform venipuncture and phlebotomy procedures.
Conduct patient assessments, triage, and ongoing monitoring.
Educate patients and families regarding treatment plans and disease management.
Document patient care using Epic EMR.
Collaborate with interdisciplinary teams including Physical Therapy, Social Services, IV Teams, and Phlebotomy Services.

Requirements

Active NH LPN/LVN License required.
Current BLS Certification required.
Minimum 1–2 years of nursing experience required.
Home Health experience preferred; Med-Surg, Long-Term Care, Skilled Nursing Facility, or Nursing Home experience accepted.
Phlebotomy and venipuncture skills required.
Epic charting experience required.
Travel experience required.
Must have a reliable personal vehicle for patient home visits.
