Job Responsibilities

Provide skilled nursing care in patients' homes
Complete patient assessments and develop care plans
Administer medications and treatments
Perform wound care and IV therapy as ordered
Educate patients and families on disease management and home care
Coordinate care with physicians and interdisciplinary teams
Maintain accurate clinical documentation

Requirements
Active Illinois RN License
Current BLS Certification
Minimum 1 year of professional nursing experience within the last 3 years in an acute care setting
Home Health experience preferred
IV Therapy, ER, or Oncology experience preferred
Cardiac or Wound Care certification preferred

Important Notes
No local candidates within 50 miles
Mileage between patient visits is reimbursed
First-time travelers accepted
