Minimum Qualifications

• Active Registered Nurse (RN) License
• Minimum of 2 years of Case Management experience
• Experience with:
  • Care Coordination
  • Discharge Planning
  • Utilization Management
  • Patient Advocacy
  • Interdisciplinary Team Collaboration
• Proficiency with Epic EMR preferred
• Strong communication and organizational skills

Traveler Requirements

• First-time travelers may be considered with approval from the Clinical Team.

Key Responsibilities

• Coordinate and oversee patient care plans throughout hospitalization.
• Collaborate with physicians, nurses, social workers, and ancillary departments to ensure appropriate care transitions.
• Conduct patient assessments and identify discharge needs and barriers.
• Facilitate safe and timely discharges to appropriate levels of care.
• Coordinate post-acute services, community resources, and follow-up care.
• Monitor resource utilization and support quality patient outcomes.
• Maintain accurate and timely documentation within Epic.
• Advocate for patients and families while ensuring compliance with regulatory and facility standards.
