Roles & Responsibilities

• Assess patients' medical, psychosocial, and discharge planning needs.
• Develop and coordinate individualized care plans.
• Collaborate with physicians, nurses, social workers, and other healthcare professionals.
• Facilitate discharge planning and transitions of care.
• Coordinate post-acute services such as home health, rehab, SNF, or DME.
• Monitor patient progress and adjust care plans as needed.
• Ensure appropriate resource utilization and length of stay management.
• Advocate for patients and families throughout hospitalization.
• Maintain accurate documentation in Meditech and other case management systems.
• Support quality outcomes, patient satisfaction, and cost-effective care.

Required Qualifications

• Active RN License.
• BLS Certification.
• Acute Care Hospital Case Management experience.
• Strong discharge planning and care coordination skills.
• Experience working in a fast-paced hospital environment.
• Excellent communication, organizational, and multitasking abilities.
• Ability to work independently and as part of a multidisciplinary team.
• Adaptable, reliable, and positive attitude.
