Required Experience

✔ Acute Care Hospital Experience Required
✔ RN Case Management Experience Preferred

Required Licensure & Certifications

✔ Active RN License

Key Responsibilities

• Coordinate patient care throughout hospitalization
• Develop and implement discharge plans
• Collaborate with physicians, nursing staff, social workers, and ancillary departments
• Facilitate transitions of care
• Assess patient needs and available resources
• Coordinate post-acute care services
• Document case management activities in Meditech, Midas, and WellSky
• Monitor patient progression and barriers to discharge
