Position Details
- Position: RN Case Manager
- Facility: Providence Saint Mary Medical Center
- Location: 18300 US Highway 18, Apple Valley, CA 92307
- Start Date: 07/13/2026
- Duration: 13 Weeks
- Schedule: 5x8 Days (8:00 AM – 4:30 PM)
- Patient Ratio: Up to 25 patients
- Charting: Epic
- Weekend Requirement: Every other weekend as needed
Requirements
- Active CA RN License
- Minimum 2 years of Acute Care Case Management/Discharge Planning experience
- Hospital Case Management experience required (Behavioral Health experience alone will not qualify)
- Travel experience required (first-time travelers not accepted)
- Experience with:
- Care Coordination & Discharge Planning
- Utilization Review
- DME, SNF Placement & Transportation Arrangements
- Insurance Plans (HMO, IPA, etc.)

Cons
Staff constantly switched. My documents weren't sent to the school, resulting in a months lost pay. It is IMPOSSIBLE to get on touch with anyone to answer questions. I have been attempting to reach them for over a month with absolutely no response! They lose paperwork (tests, immunizations, background checks and fingerprints) resulting in even more lost work.
