Shift Notes:

start 8:00 start at 7:00 or 9:00 if they would like 5x8

Weekend Requirements:

Would prefer some weekend coverage ~1/month, but would be open to discuss with candidate

• recent / new grads accepted
• At least 6 months of previous PT experience, NOT required, but; [preferred]
• travel experience preferred, not required -- NEW travelers accepted!
• BLS [required]
• inpatient setting
  • State license [required]
  • Not an employee of UnityPoint in the last 12 months [required]

• Profile should include

  ·             Driver’s License [required]

  ·        Work history/resume [required]

  ·        Skills checklist (must have been completed within 1 year of submission date) [required]

  ·        License verification [required]

  ·        Copy of required certifications

  ·        References - 1 supervisory reference required at time of submission [required]

  **Must have taxable address outside of 50 miles from facility. **

  Unless Madison where they will accept local with a 10% rate reduction.
