We are seeking an experienced RN Case Manager to join a leading Acute Care Hospital for a 13-week assignment. The ideal candidate will have a strong background in inpatient case management, care coordination, discharge planning, and utilization management within an acute care hospital setting.

Minimum Qualifications

• Active RN License (Virginia License or Compact License)
• Pending Virginia License Accepted (Must be active before start date)
• Minimum of 2 years of recent Acute Care Hospital Inpatient Case Management experience
• Experience with:
  • Care Coordination
  • Discharge Planning
  • Utilization Review
  • Interdisciplinary Care Management
• Epic EMR experience required
• Strong communication and organizational skills

Key Responsibilities

• Coordinate patient care throughout hospitalization and discharge.
• Assess patient needs and identify barriers to discharge.
• Collaborate with physicians, nursing staff, social workers, and ancillary departments.
• Develop and implement individualized discharge plans.
• Facilitate transitions of care to post-acute facilities, home health agencies, and community resources.
• Perform utilization management activities to ensure appropriate level of care and resource utilization.
• Maintain accurate and timely documentation in Epic.
• Advocate for patients while supporting quality and regulatory standards.
