We are seeking an experienced RN Case Manager to join a dynamic Acute Care Hospital team. This role is responsible for coordinating patient care throughout the continuum, facilitating safe and timely discharges, and ensuring compliance with regulatory requirements while optimizing patient outcomes.

Minimum Qualifications

• Active Registered Nurse (RN) License
• Minimum 3 years of recent Acute Care Hospital Case Management experience
• Previous travel experience required (First-time travelers will not be considered)
• Strong experience with:
  • Care Coordination
  • Discharge Planning
  • Utilization Management
  • Length of Stay Management
  • Medicare Regulations and Compliance
  • Interdisciplinary Care Management

Key Responsibilities

• Assess, evaluate, and coordinate patient discharge plans.
• Facilitate transitions to the most appropriate level of care.
• Coordinate services and resources to ensure continuity of care.
• Monitor patient progress and treatment plans daily.
• Identify and address barriers to discharge.
• Track and manage patient length of stay to support efficient care delivery.
• Collaborate with physicians, nursing staff, social workers, therapists, and ancillary departments.
• Ensure compliance with Medicare regulations and patient rights requirements.
• Provide and document:
  • Important Message from Medicare (IMM)
  • Medicare Outpatient Observation Notice (MOON)
  • Patient choice documentation
• Maintain accurate and timely case management documentation.
