We are seeking an experienced Registered Nurse Case Manager to support a new Transitional Care Management (TCM) program. This role focuses on coordinating patient care after hospital discharge, reducing readmissions, and improving patient outcomes through proactive outreach and care coordination.

Requirements:

• Active RN License
• BLS Certification
• Epic experience required
• Experience in Case Management, Care Coordination, or Transitional Care
• Strong knowledge of Medicare guidelines, HIPAA, and discharge planning

Key Responsibilities:

• Conduct post-discharge outreach within required timeframes
• Complete medication reconciliation and discharge assessments
• Coordinate follow-up appointments with PCPs and specialists
• Identify barriers to care and assess social determinants of health
• Monitor high-risk patients and escalate clinical concerns
• Collaborate with providers, social workers, and community resources
• Maintain accurate TCM documentation in Epic

• Transitional Care Management (TCM)
• Annual Wellness Visits (AWV)
• Care Coordination and Population Health
• Value-Based Care programs
