Position Overview
Ardor Health Solutions is seeking an experienced Home Health Physical Therapist for a 13-week travel assignment with CenterWell Home Health in Flint, Michigan. This position offers the opportunity to provide personalized, one-on-one care to patients in their homes while helping them regain mobility, maximize independence, and improve their quality of life.
Job Responsibilities
- Complete comprehensive physical therapy evaluations in patients' homes.
- Develop and implement individualized plans of care.
- Treat patients recovering from orthopedic procedures, neurological conditions, cardiopulmonary diagnoses, and generalized weakness.
- Educate patients and caregivers on home exercise programs, mobility, safety, and fall prevention.
- Collaborate with physicians, nurses, occupational therapists, speech therapists, and case managers.
- Complete timely and accurate documentation using Homecare Homebase (HCHB).
- Travel throughout the assigned coverage area to provide exceptional patient care.
Qualifications
- Active Michigan Physical Therapist license (or ability to obtain before start)
- Minimum of 1 year of recent Home Health experience
- OASIS documentation experience required
- Homecare Homebase (HCHB) experience strongly preferred
- Current BLS certification
- Valid driver's license, reliable transportation, and current auto insurance
- Strong communication, organization, and time management skills
