RN Case Manager | Atlanta, GA

Now Hiring: Case Manager for a leading healthcare facility in Atlanta!

Location: Atlanta, Georgia
Facility: Piedmont
Contract Length: Travel/Contract Opportunity
Shift: Days | 8-Hour Shifts

Requirements:
• Minimum 3 years of Case Management experience
• BLS Certification required
• Strong discharge planning and care coordination experience

Responsibilities:
• Assess, evaluate, and drive patient discharge plans
• Coordinate services to ensure patients receive the appropriate level of care
• Initiate, evaluate, and re-evaluate treatment plans daily
• Monitor patient progress and identify barriers to discharge
• Manage length of stay and support efficient care transitions
• Ensure compliance with Medicare regulations, including:

IMM Notices
MOON Notices
Patient choice requirements

Ideal Candidate:
• Strong critical thinking and communication skills
• Experience collaborating with interdisciplinary teams
• Knowledge of utilization review and discharge planning processes
• Comfortable working in a fast-paced acute care environment
