Description
Years of experience: 2-year acute hospital case management experience required.
Will be required to watch up to 2 units on the weekend and focus on discharge planning
• Weekend REQ: Every other weekend. Must be available for holiday coverage
Travel experience required
Patient ratio:1:35
Charting system- Epic
Certification required - BLS, MSW (Master of social worker)
