Social Worker (LCSW/LPC)

Schedule: Full-Time, 40 hours/week (7:30 AM – 6:00 PM)
Contract Length: 26 Weeks

Position Summary

The Medical Social Worker collaborates with the multidisciplinary healthcare team to assess, plan, implement, and evaluate psychosocial services for patients whose social, emotional, behavioral, or financial challenges may impact their medical treatment and recovery. Services are provided in accordance with physician orders, professional standards of practice, and established care plans.

Essential Responsibilities

• Conduct comprehensive psychosocial assessments to identify patient needs and barriers to care.
• Develop and implement individualized intervention plans to support treatment goals.
• Collaborate with physicians, nurses, therapists, and other healthcare professionals to coordinate patient care.
• Provide counseling, crisis intervention, emotional support, and resource coordination.
• Assist patients and families in planning for current and future psychosocial needs.
• Facilitate referrals to community resources and support services as appropriate.
• Provide financial counseling and assistance in accessing available benefits and resources.
• Monitor patient progress and communicate changes in status to the healthcare team.
• Maintain accurate and timely documentation in accordance with company policies and regulatory requirements.
• Participate in quality improvement initiatives and interdisciplinary team meetings.
• Serve as a resource to healthcare staff regarding psychosocial factors affecting patient care and recovery.

Required:

• Current LCSW or LPC license in WI
• BLS
• Minimum one year of social work experience in a healthcare, behavioral health, or related setting within the past two years.
• Experience with case management, discharge planning, crisis intervention, or behavioral health services.
