Medical Social Worker – Job Description Summary

The Medical Social Worker (MSW) works collaboratively with physicians and the multidisciplinary healthcare team to support patients whose social, emotional, financial, or environmental challenges may impact their medical treatment, recovery, or overall well-being. Services are provided based on physician orders and in accordance with established care plans, company policies, and regulatory requirements.

Key Responsibilities

• Conduct psychosocial assessments and identify patient needs.
• Develop, implement, evaluate, and modify care interventions to support patient goals.
• Collaborate with physicians, nurses, therapists, and other healthcare team members.
• Provide counseling and support to patients and families coping with illness, disability, or terminal conditions.
• Assist with discharge planning and future care needs.
• Coordinate referrals to community resources and support agencies.
• Provide financial counseling and guidance related to treatment needs.
• Monitor patient progress and communicate changes to the healthcare team.
• Serve as a resource and educator regarding psychosocial factors affecting health outcomes.
• Maintain knowledge of Medicare, Medicaid, and other reimbursement programs.
• Participate in quality improvement initiatives and maintain accurate documentation.
• Ensure compliance with HIPAA, company policies, and professional standards.

Qualifications

• LCSW in WI
• At least 1 year of healthcare social work experience preferred.
• BLS

Assignment Details

• Schedule: Monday–Friday, 7:45 AM–4:30 PM
• Duration: 26-week assignment
• Hours: 40 hours per week
• Some flexibility in scheduling may be required.
