Description
We are seeking a compassionate and experienced Home Health Registered Nurse to provide skilled nursing care to patients in their homes in San Diego, CA.
Key Responsibilities
- Perform skilled nursing visits and comprehensive in-home patient assessments.
- Complete OASIS Start of Care (SOC), Resumption of Care (ROC), Recertification, and Discharge assessments.
- Develop, implement, and update individualized care plans.
- Administer medications, wound care, IV therapy, and other skilled nursing treatments as ordered.
- Educate patients and caregivers on disease management, medications, and home safety.
- Coordinate care with physicians, therapists, case managers, and other healthcare professionals.
- Maintain accurate and timely documentation in the EMR.
Minimum Requirements
- Minimum 1 year of recent Home Health RN experience
- Experience caring for geriatric patients
- Current BLS/CPR Certification
- OASIS experience required
- Active California RN License preferred (pending CA license accepted if application has been submitted prior to submission)
- Valid Driver's License, reliable transportation, and current auto insurance
