Description

We are seeking a compassionate and experienced Home Health Registered Nurse to provide skilled nursing care to patients in their homes in San Diego, CA.

Key Responsibilities

Perform skilled nursing visits and comprehensive in-home patient assessments.
Complete OASIS Start of Care (SOC), Resumption of Care (ROC), Recertification, and Discharge assessments.
Develop, implement, and update individualized care plans.
Administer medications, wound care, IV therapy, and other skilled nursing treatments as ordered.
Educate patients and caregivers on disease management, medications, and home safety.
Coordinate care with physicians, therapists, case managers, and other healthcare professionals.
Maintain accurate and timely documentation in the EMR.

Minimum Requirements

Minimum 1 year of recent Home Health RN experience
Experience caring for geriatric patients
Current BLS/CPR Certification
OASIS experience required
Active California RN License preferred (pending CA license accepted if application has been submitted prior to submission)
Valid Driver's License, reliable transportation, and current auto insurance

The ideal candidate is a compassionate and organized Home Health RN with recent OASIS experience who enjoys providing personalized, high-quality care in patients' homes while effectively managing an independent schedule.
