Description
We are seeking an experienced RN Case Manager to join a fast-paced Acute Care Hospital setting. The Case Manager will be responsible for coordinating patient care throughout the hospitalization process, ensuring appropriate utilization of resources, facilitating safe and timely discharges, and reducing avoidable admissions and readmissions.
Requirements
- Active California RN License
- Current BLS Certification
- Minimum 1+ years of RN Case Management experience
- Recent Inpatient Acute Care Hospital Case Management experience is required
- Strong background in Care Coordination, Discharge Planning, and Utilization Management
- Knowledge of CMS, Joint Commission, and regulatory requirements
