JOB TITLE: Pharmacist
POSITION SUMMARY:
The Pharmacist is responsible for providing comprehensive pharmaceutical care in both outpatient and inpatient settings, including emergency room support when needed. This role requires a flexible, team-oriented professional who can efficiently process prescriptions, provide patient counseling, manage controlled substances in compliance with federal and state regulations, and collaborate with interdisciplinary healthcare teams to deliver safe and effective patient care. The Pharmacist must be willing to work variable shifts, including days, evenings, nights, weekends, and holidays.
ABOUT THE PHARMACY DEPARTMENT:
We are a Tribal 638 healthcare facility serving the San Carlos Apache community. Our hospital is a growing and progressive organization that fills approximately 1,000 prescriptions daily utilizing advanced pharmacy technology, including robotic dispensing systems and automated counting devices. Our pharmacy team supports inpatient and outpatient services and actively participates in community outreach programs, specialty clinics, and educational initiatives. Pharmacists have opportunities for professional growth, preceptorship, and meaningful service to an underserved population.
POSITION RESPONSIBILITIES:
• Accurately review, verify, and dispense medications in accordance with provider orders and applicable laws and regulations.
• Process outpatient prescriptions efficiently while maintaining the highest standards of medication safety and accuracy.
• Provide pharmaceutical care and clinical support for inpatient and emergency department patients as needed.
• Review medication profiles for appropriateness, drug interactions, contraindications, and therapeutic effectiveness.
• Counsel patients regarding medication use, side effects, dosing instructions, and disease management in a timely and professional manner.
• Process and monitor controlled substances in compliance with DEA regulations, organizational policies, and applicable laws.
• Collaborate with physicians, nurses, and interdisciplinary healthcare teams to optimize patient outcomes.
• Participate in medication therapy management, vaccine services, anticoagulation management, and other clinical pharmacy initiatives.
• Provide drug information and recommendations to healthcare providers and staff.
• Assist with medication reconciliation and transitions of care activities.
• Ensure compliance with pharmacy policies, regulatory standards, and accreditation requirements.
• Utilize pharmacy information systems and automated dispensing technologies, including ScriptPro Pharmacy Management System, SP200 Robot, Omnicell Automated Dispensing Cabinets (ADCs), Carousel systems, and Kirby Lester counting devices.
• Document pharmaceutical interventions and patient care activities in the Electronic Health Record (EHR).
• Participate in departmental orientation, continuing education, quality assurance initiatives, and performance improvement activities.
• Precept pharmacy students, pharmacy technicians, and pharmacy interns as assigned.
• Participate in community outreach and health education initiatives.
• Perform additional duties as assigned.
SCHEDULE:
• Variable shifts, including days, evenings, nights, weekends, and holidays.
• Flexible scheduling based on departmental needs.
• No on-call requirements.
MINIMUM QUALIFICATIONS:
• Bachelor of Science (BS) in Pharmacy and/or Doctor of Pharmacy (PharmD) from an accredited School of Pharmacy.
• Current and active, unrestricted Pharmacist license required; Arizona Pharmacist license preferred.
• Basic Life Support (BLS) certification required.
• Advanced Cardiovascular Life Support (ACLS) certification required for inpatient and emergency department coverage.
• Pediatric Advanced Life Support (PALS) certification preferred and may be obtained on-site.
• Minimum of two (2) years of clinical pharmacy experience preferred.
• Hospital pharmacy experience required.
• Experience in both outpatient and inpatient pharmacy settings strongly preferred.
• Ability to work variable shifts and adapt to changing operational needs.
PREFERRED QUALIFICATIONS:
• PharmD degree preferred.
• PGY1 Pharmacy Residency preferred.
• Experience with automated dispensing systems and electronic health record systems.
• Experience in ambulatory care, vaccine services, anticoagulation management, and community outreach programs preferred.
