Description
We are looking for experienced Travel Registered Nurse (RN) to provide high-quality patient care in the home health setting. The RN will deliver skilled nursing services, educate patients and families, coordinate care plans, and collaborate with interdisciplinary teams to ensure optimal patient outcomes.
This role is ideal for nurses who enjoy autonomy, building strong patient relationships, and providing care in the comfort of patients' homes.
Schedule & Coverage
- Weekly Hour Guarantee: 32–36 hours
- Shift Schedule: Monday through Friday
- Coverage Area: Fort Wayne, IN and surrounding communities
- On-Call/Weekend Requirement: Yes
- Mileage Reimbursement: Yes, mileage is reimbursed between patient visits (first patient to last patient)
Travel Pay - $2,526/ weekly Gross
Key Responsibilities
- Provide comprehensive skilled nursing care to patients in their homes.
- Perform patient assessments and develop individualized care plans.
- Administer medications and treatments as ordered by physicians.
- Monitor patient conditions and report changes to the interdisciplinary team.
- Educate patients and caregivers regarding diagnoses, medications, disease management, and home safety.
- Complete accurate and timely clinical documentation in accordance with agency and regulatory standards.
- Coordinate care with physicians, therapists, social workers, and other healthcare professionals.
- Participate in on-call and weekend rotations as required.
- Ensure compliance with all state, federal, and agency policies and procedures.
Required Qualifications
- Active Registered Nurse (RN) license in good standing.
- Current and unrestricted RN certification/licensure.
- Ability to travel throughout the Fort Wayne service area.
- Strong assessment, communication, and patient education skills.
- Valid driver's license, reliable transportation, and current auto insurance.
Preferred Qualifications
Candidates who possess the following will stand out:
- Previous home health nursing experience.
- Experience managing patients independently in community settings.
- Knowledge of OASIS documentation and Medicare home health regulations.
- Strong time-management and organizational skills.
- Ability to build rapport with patients and families.
Required Certification
- Active Registered Nurse (RN) license.
