Travel Physical Therapist (PT) – Home Health

BrightSpring Health Services is seeking a Travel Physical Therapist for a 13-week Home Health contract in beautiful Grand Junction, Colorado. Join a supportive home health team providing one-on-one care to adult and geriatric patients in the comfort of their own homes while helping them regain mobility, independence, and confidence.

This is an excellent opportunity for therapists who enjoy autonomy, flexible scheduling, and building meaningful relationships with patients throughout their recovery journey.

Position Highlights

Home Health
13-Week Travel Contract
Monday–Friday Schedule (8:00 AM – 5:00 PM)
Adult & Geriatric Patient Population
Mesa & Garfield County Coverage Area
Mileage Reimbursement Provided
Collaborative Home Health Team

Responsibilities

Perform comprehensive physical therapy evaluations and reassessments in patients' homes
Develop individualized treatment plans to maximize mobility and functional independence
Treat patients with orthopedic, neurological, post-surgical, and medically complex conditions
Provide gait training, balance training, therapeutic exercise, transfer training, and home safety assessments
Educate patients and caregivers on home exercise programs, fall prevention, and equipment use
Coordinate care with physicians, nurses, case managers, and other healthcare professionals
Complete timely documentation and OASIS assessments as required
