Description
Physical Therapist (PT) – Home Health
Facility Type: Home Health
Must Have
- Active Physical Therapist License
- BLS Certification
- Home Health Physical Therapy experience
- Experience performing patient evaluations and functional assessments
- Experience developing individualized treatment plans
- Knowledge of home safety assessments
- Strong clinical judgment and problem-solving skills
- Excellent patient and caregiver education skills
- Ability to work independently and manage a daily caseload
- Valid driver's license and reliable transportation (typically required for Home Health)
Preferred
- Previous Home Health travel experience
- Experience treating adult and geriatric patients
- OASIS documentation experience
- Experience with electronic medical records (EMR)
Schedule
- Day Shift: 8 Hours
- Monday–Friday
- Flexible Schedule: Sunday–Thursday or Tuesday–Saturday
- 40 Guaranteed Hours per Week
- Contract Length: 13 Weeks
Key Responsibilities
- Perform comprehensive physical therapy evaluations in patients' homes
- Develop and implement individualized treatment plans
- Provide therapeutic exercises, gait training, balance training, and mobility training
- Assess home safety and recommend adaptive equipment
- Educate patients and caregivers on treatment plans and home exercise programs
- Monitor patient progress and modify treatment plans as needed
- Collaborate with physicians, nurses, occupational therapists, speech therapists, and case managers
- Complete timely and accurate clinical documentation
- Promote functional independence and prevent hospital readmissions
- Ensure compliance with Home Health regulations and agency policies
