Job Title: Pharmacist – Diabetes Care (Contract)

Contract Duration: July 27, 2026 – January 23, 2027
Employment Type: Full-Time Contract
 

Position Overview

We are seeking a dedicated and experienced Pharmacist – Diabetes Care to provide comprehensive pharmaceutical services for patients with diabetes and other chronic conditions. The ideal candidate will work collaboratively with physicians, nurses, dietitians, and other healthcare professionals to optimize medication therapy, improve patient outcomes, and promote safe, evidence-based diabetes management.

This position requires excellent clinical knowledge, strong patient counseling skills, and the ability to deliver high-quality pharmaceutical care in a multidisciplinary healthcare setting.
 

Key Responsibilities

  • Provide clinical pharmacy services focused on diabetes management and chronic disease care.
  • Review, verify, and dispense medications in accordance with federal, state, and organizational regulations.
  • Assess medication regimens for safety, effectiveness, drug interactions, and therapeutic appropriateness.
  • Educate patients on diabetes medications, insulin administration, glucose monitoring, lifestyle modifications, and medication adherence.
  • Collaborate with healthcare providers to develop and optimize individualized treatment plans.
  • Monitor patient outcomes and recommend therapy adjustments based on clinical guidelines.
  • Maintain accurate medication records and clinical documentation within the electronic health record (EHR).
  • Participate in medication reconciliation during patient admissions, transfers, and discharges.
  • Provide drug information and consultation to healthcare professionals.
  • Ensure compliance with pharmacy policies, accreditation standards, and regulatory requirements.
  • Participate in quality improvement initiatives and patient safety programs.
  • Assist with inventory management, medication storage, and controlled substance accountability.

Required Qualifications

  • Doctor of Pharmacy (PharmD) or Bachelor of Science in Pharmacy from an accredited institution.
  • Active, unrestricted Pharmacist license in the applicable state.
  • Minimum of 2 years of clinical pharmacist experience, preferably in diabetes or ambulatory care.
  • Strong knowledge of diabetes pharmacotherapy, insulin management, and current clinical practice guidelines.
  • Experience with Electronic Health Record (EHR) systems.
  • Excellent communication, patient education, and interpersonal skills.
  • Ability to work independently and collaboratively within an interdisciplinary healthcare team.

Preferred Qualifications

  • Board Certification in Ambulatory Care (BCACP) or Pharmacotherapy (BCPS).
  • Certified Diabetes Care and Education Specialist (CDCES) preferred.
  • Previous experience in Indian Health Service (IHS), Tribal Health, VA, or community health settings.
  • Experience with chronic disease management programs.

Required Certifications & Licensure

  • Active Pharmacist License
  • Basic Life Support (BLS) certification (if required by facility)
  • Board certification or diabetes certification preferred
