Travel Registered Nurse (RN) – Long Term Care (LTC) / Skilled Nursing Facility

Job Summary

We are seeking an experienced Long Term Care (LTC) Registered Nurse (RN) for a travel assignment in a skilled nursing facility attached to a hospital. The RN will provide comprehensive care for elderly and rehabilitation patients, including assessments, medication administration, clinical interventions, and coordination with interdisciplinary teams. This role requires strong clinical judgment, the ability to manage high patient loads, and experience working with geriatric populations.
 

Facility Overview

  • 70-bed nursing facility
  • Majority private rooms
  • Attached to hospital system
  • Strong interdisciplinary support (therapy, social work, dietary, activities, administration)
  • On-site pharmacy and medication dispensing system

Responsibilities

  • Provide direct nursing care to geriatric and long-term care residents.
  • Perform patient assessments and respond to changes in condition.
  • Administer medications and treatments per physician orders.
  • Perform skilled nursing procedures including IV therapy, catheter care, and ostomy care.
  • Maintain accurate documentation using facility EMR (ECS system).
  • Participate in care planning and coordination with interdisciplinary team.
  • Supervise and support CNAs and LPNs as needed.
  • Ensure patient safety, infection control, and regulatory compliance.
  • Provide emotional and physical support to patients and families.

  • Elderly residents
  • Dementia and memory care patients
  • Frail and medically complex long-term care residents
  • Rehabilitation patients

Required Qualifications

  • Active Registered Nurse (RN) License
  • Current BLS (AHA) certification
  • Experience in Long Term Care (LTC) or Skilled Nursing Facility (SNF) settings
  • Strong geriatric nursing experience
  • Ability to manage high patient ratios (up to 1:20)

Schedule & Shift Details

  • 12-hour night shifts
  • Every other weekend required
  • Must work at least half of assigned holidays
