Travel Physical Therapist (PT) – Home Health

BrightSpring Health Services is seeking a Travel Physical Therapist for a 13-week Home Health contract in Cullman, Alabama. Join a respected home health team providing one-on-one care to adult and geriatric patients in the comfort of their homes while helping them regain mobility, independence, and confidence.

Responsibilities
- Perform comprehensive physical therapy evaluations and reassessments.
- Develop individualized treatment plans based on each patient's functional goals.
- Treat patients with orthopedic, neurological, post-surgical, and medically complex conditions in the home setting.
- Provide gait training, balance training, therapeutic exercise, transfer training, and fall prevention education.
- Collaborate with physicians, nurses, case managers, and the interdisciplinary care team.
- Educate patients and caregivers on home exercise programs and safety strategies.
- Complete timely EMR documentation and progress reporting.
